1) What is the difference between the Student Health Record (SHR)& The Athletic Participation Form (APF)?

• The SHR is a physical exam form which is required on admission, and also prior to returning to 4th, 6th, 9th and 11th grades. Students can not attend school without submitting this form.
• The APF is required for participation in middle and high school for extra-curricular sports activities (including cheerleading). A new physical exam must be performed each year prior to the student’s participation in the extracurricular activities.
