2) How often does my child need to have a Physical Exam (PE)?
• A physical exam must be done on admission, and prior to 4th, 6th, 9th and 11th grade. A NEW TB test must be done at this time. Exam results must be documented on the Student Health Record (SHR).
• Middle & High School students must also have a Physical Exam to participate in Extra-curricular Sports Activities. These exams are good for 1 calendar year. The results of this exam must be documented on the Athletic Participation Form (APF).
Students entering 6th grade must provide proof of having had the 3 part Hepatitis B vaccination and have had either the Varicella (Chicken Pox) vaccination or provide M.D. documented proof of having had chicken pox disease.
