What is the procedure if my child is on medication that needs to
be received at school? |
| Elementary
School - Please attempt to time
medication administration outside of school hours. If your child
is on medication (both prescription & Over-The-Counter) that
needs to be taken at school, a Request For Administration
of Medicine - Elementary School form (RAMF-ES) must be
filled out and signed by the doctor AND the parent. For elementary
students, the medicine must be stored in the Health Room and administered
by the Health Services staff. Certain approved emergency medications
such as Albuterol Inhalers and Epi-pens, may be carried and self-administered
by the child IF approved by the child's physician and parents/guardians
who feel that the child: 1) Has been properly trained and 2) Has
the capability and maturity to correctly and appropriately self-administer
the medication. In this case as well, a fully completed RAMF must
be submitted to the Health Services Department. The RAMF must
be resubmitted yearly. Medicines that are controlled substances
may only be stored and administered by Health Services Personnel.
Middle/High
School -
Please attempt to time medication administration outside of school
hours. If your child is on medication (both prescription &
Over-The-Counter) that needs to be taken at school, a Request
For Administration of Medicine - Middle/High School form
(RAMF-MS/HS) must be filled out and signed by the doctor AND the
parent. For middle & high school students, the medicine may
be either stored in the Health Room and administered by the Health
Services staff, or in certain instances self-carried and self-administered.
In order to self-carry/administer one's medicine, the child's
physician and parents/guardians who feel that the child: 1) Has
been properly trained and 2) Has the capability and maturity to
correctly and appropriately self-administer the medication must
give written approval. In this case as well, a fully completed
RAMF must be submitted to the Health Services Department. The
RAMF must be resubmitted yearly. Medicines that are controlled
substances may only be stored and administered by Health Services
